Birmingham OwnHealth strives to complement and integrate with existing local healthcare activities to make self-care a reality for people living with long term conditions (LTCs). If this is to be achieved, we must bring together:
- expertise in care management from NHS Direct
- experience of Pfizer Health Solutions in delivering disease management solutions
- existing NHS services
- skills of specially trained care managers
- proven clinical support software to ensure consistent advice.
