How to join


Why should I join?

How to join

If you have a GP in east or north Birmingham or Walsall, you may be able to become a member of the OwnHealth service.

If you have a GP in east or north Birmingham

If you are aged over 18, can use a telephone and come under one or more of the following categories, you may be able to become a member of the Birmingham OwnHealth service:

  • diabetes

  • coronary heart disease
  • heart failure
  • chronic obstructive pulmonary disease (COPD)
  • stroke or TIA (‘mini-stroke’)
  • chronic kidney disease
  • high blood pressure
  • aged over 65 and would like help with a range or health conditions.

To join Birmingham OwnHealth, print off the application form below and post or fax it to the address on the form.

Alternatively, you can ask your NHS Birmingham East and North GP or other health professional to refer you to the service.

If you have a GP in Walsall

If you are aged over 18, can use a telephone and come under one or more of the following categories, you may be able to become a member of the Walsall OwnHealth service:

  • diabetes

  • coronary heart disease
  • cardiovascular disease (CVD).

To join Walsall OwnHealth, print off the application form below and post or fax it to the address on the form.
