About us
Birmingham OwnHealth is a ground-breaking healthcare service for people with long term conditions in Birmingham. It provides one-to-one advice and support to members in several languages, including English and Punjabi, working with a designated care manager over the telephone to help manage their condition.
If you are aged over 18, can use a telephone and come under one or more of the following categories, you may be able to become a member of the Birmingham OwnHealth service:
- diabetes, type 1 and 2
- coronary heart disease
- heart failure
- chronic obstructive pulmonary disease (COPD), such as emphysema or chronic bronchitis
- stroke or TIA (‘mini-stroke’)
- chronic kidney disease
- high blood pressure
- over 65 and would like help with a range or health conditions
We know that living with conditions like these can have a negative impact on your everyday life and that it can sometimes feel like you have little or no say when it comes to your treatment or care. But it doesn’t have to be like this.
Birmingham OwnHealth helps people with these conditions to have more independent lives, make positive changes to their health and lifestyles and give them the power to manage their conditions better. You play a much bigger role in the delivery of your care and you are never more than a phone call away from all the information, support and encouragement you need.
Feedback shows that Birmingham OwnHealth is already improving the health of members:
- They have made positive changes to their health, including having a better diet, taking more exercise and quitting smoking
- They had improved their weight and their cholesterol, blood pressure and blood glucose levels
- They didn’t need to go to their GP or hospital as often
Birmingham OwnHealth will be expanded over the next few years to benefit more members in Birmingham. The service will also use the latest health technologies, such as health monitoring devices for members to use in their own home and share their results with a health professional. Closer working between health and social care is also planned.
